Not only are vital signs written in a vital signs tab, but vital signs can be written in a nurses' note, progress note, and history and physical. Pulse oximetry readings can be written to include a client on room air, oxygen, or mechanical ventilation. The last tab we have is the laboratory results tab. For current NCLEX items, there are specific laboratory values that the candidate is expected to know. These laboratory values are listed in the test plan. As we continue moving forward towards the next- generation NCLEX, more information will be available to assist candidates in preparing to take the exam.

Detecting Bias A test is biased if it is less valid in measuring a test construct in one group than it is in another (Shealy & Stout, 1993). A biased NCLEX would be a test that measures nursing knowledge in one group of examinees and some unrelated construct, such as reading comprehension, in another.

You have to figure that out. So there is a move in the NCLEX to be more authentic, and authenticity is really important. If we have to do a computerized test versus a practicum that we would grade graduates on, then that has to be... that licensure exam has to be as authentic as possible to represent actual practice.

It is related to the mission of organization and examinations. For us, the mission is for public protection. So, that's why, when we set a standard, we need to keep that mission in our mind. What standard going to help you protect the public? And the standard should be revisited when the test specifications, scope of practice, education programs, and other factors change. That's why, every time, when we change our test plan, when we change some scope of practice, we need to re-evaluate our standard. And I can imagine that when the NGN comes, there's a big piece about standard setting.

Panel of SMEs A panel of 10 RNs was assembled to assist with the practice analysis. Panel members worked with, educated and/or supervised the practice of RNs within their first 12 months of practice or were themselves newly licensed RNs. Panel members represented geographic areas of the U.S. territories/jurisdictions using the NCLEX for licensure decisions, all major nursing specialties and varied practice settings. The panel used the current test plan category structure describing the types of activities performed by newly licensed RNs and developed a list of activities performed within each category of the structure.

So then you're going to ask yourself, "How does a nurse practice observing?" Because we want to make sure we bring it back to the real world. Remember, we're not just preparing them to recognize cues on the test question or the NCLEX exam, but more importantly, we're preparing them for actual nursing practice. So what do nurses do when they practice observation skills? Well, nurses use noticing an observation when they monitor a patient for a complication, for example, or when they're completing a focused assessment.
