And these are really pervasive concepts, ideas, things that we believe cross all of the domains. And people will say, "Well, health policy should have been a domain." That, we got a lot of feedback on while we were developing. Also, communication, I think, were the two. And then social determinants of health. But those concepts are embedded strongly across all of the domains and, we felt, were equally or more important than many of the domains because they are so pervasive and cross all of the domains. And these are the four spheres of care that I referred to.

This capacity to manipulate the electronic record makes it far too easy for trainees to avoid taking their own histories and coming to their own conclusions about what might be wrong. Senior physicians also cut and paste from their own notes, filling each note with the identical medical history, family history, social history, and review of systems. Writing in a personal and independent way forces us to think and formulate our ideas. Notes that are meant to be focused and selective have become voluminous and templated, distracting from the key cognitive work of providing care.

Hands up high. Oh, quite a few. And put it in the chat if you're virtual. And let me ask you, how many people have been the victim of fraud? Raise your hands up high. Don't be embarrassed. Yeah. There's a Kathleen Russell in California that used my Social Security number to get a cell phone. I didn't know it until I applied for a mortgage and this came up on my credit report. So, even though today you may not be someone that looks at licensure fraud, it's important to know about fraud and how it can happen.

Just remember, these questions went out prior to the pandemic, but even keeping that in mind, most of the responses would be the same today. One is the aging workforce and the aging population as well as more complex patients, the opioid crisis, we talked about diversity, social determinants, population health, we talked about the decreased number of primary care providers, and how that impacted supply and demand of nurse practitioners. We talked about the way care was delivered, the way consumers were informed, and that would include things like telehealth, AI, remote monitoring.
